​AX1 Survey Prize Draw – Terms & Conditions

1. Eligibility

 

The prize draw is open to qualified optometrists, ophthalmologists, and other eye care professionals (including orthoptists and dispensing opticians) aged 18 or over. Entrants must be practising or training in a relevant field. The prize draw is open internationally but subject to local regulations and device registration requirements in each country.

Employees of Occuity Ltd, their immediate family members, and any associated agencies are not eligible to enter.

2. How to Enter

 

To enter, participants must complete the official AX1 survey in full and provide valid contact details. Only one entry per person is permitted. Duplicate entries will be disqualified.

3. Competition Period

 

The prize draw opens on the date the survey is first distributed and closes on 31st July 2025 at 23:59 BST. Entries received after this time will not be valid. Winners will be drawn and notified by 1st September 2025.

​

4. Prizes

​

There are a total of 6 prizes available:

• 1 x AX1 Axiometers™ — commercial devices provided free of charge upon release in the entrant’s registered territory, subject to applicable regulatory approvals.

• 5 x £20 Amazon vouchers

​

The AX1 prize will be shipped at no cost to the winners once the device has been approved and is available in their country.

All winners will be selected via a random prize draw. No cash alternatives are available, and prizes are non-transferable. Occuity reserves the right to substitute the prize with another of equivalent value without notice, if necessary.

4.1 Non-Transferability and Resale of Prizes

Prizes are non-transferable, non-exchangeable, and may not be resold, traded, or offered for sale, including but not limited to via online auction sites or other third-party platforms. Any attempt to do so may result in the forfeiture of the prize at the sole discretion of the promoter.

 

5. Winner Notification

 

Winners will be notified by email using the details provided at the time of entry. If a winner does not respond within 14 days of being notified, Occuity reserves the right to withdraw the prize and select an alternative winner.

A list of winners (name, region, and profession only) may be published or used in future promotional material.

 

6. Use of Data

 

By entering, participants consent to:

• The use of their survey responses for internal product development and research purposes.

• Their contact details being used to notify winners and for occasional marketing communications from Occuity (you may unsubscribe at any time).

 

Survey responses will not be anonymised unless stated. Occuity will not share personal information with third parties except as required by law.

7. General

​

• The organiser of the prize draw is Occuity Ltd, The Blade, Abbey Square, Reading, Berkshire, UK.

• The competition is governed by English law, and disputes will be subject to the exclusive jurisdiction of the English courts.

• By entering, all participants agree to be bound by these terms and conditions.

8. Limitation of Liability

 

• Occuity does not accept any liability for any damage, loss, injury or disappointment suffered by any entrants as a result of either participating in the giveaway or being selected for a prize, save that Occuity does not exclude its liability for death or personal injury as a result of its own negligence.

• Occuity does not provide any form of practical or IT support for this prize. On receipt, all responsibilities relating to warranty and the product are that of the prize winner.​
